Record a decision about a pull request in its own layer, not by force-pushing the layer it describes

Why

Folding an ADR row into the PR it describes means force-pushing a branch already open for review to add a record ABOUT it. A separate layer is reviewable on its own and neither described layer has to move.

Measured

ADR 0013 and 0014 landed as layer 4 without touching layers 2 or 3. The cost: both rows sat at research-preview with their PRs unmerged and nothing moves them automatically, which is the derived stage doing its job.

This decision declares none. Every name in this section, when present, resolves against the capability vocabulary or the floor registry -- a typo fails the generator rather than rendering.
